REFERRAL PROGRAMME

​We're here to help. Contact us today for an initial consultation

  1. The applicant will be asked to fill out the form to be matched with one of our social workers.

  2. A three way meeting will be arranged between the Social Worker, M.E.L. Supported Living and Support key worker and the young person to discuss their needs and identify any potential sources of support.

  3. A representative from M.E.L. Supported Living will walk you through how we work, including a detailed explanation of the terms and conditions of our placement agreement.

  4. The young person will be asked to sign an agreement which lists our expectations of their conduct whilst placed with M.E.L. Supported Living and gives them an opportunity to acknowledge this agreement with us.

  5. One placement agreement is signed by all parties. The work will begin on the agreed start date.

WHAT HAPPENS NEXT?

The young person and their belongings will be collected by a M.E.L. Supported Living representative at the agreed time and location, and they will be transported to the placement. The Key-worker will spend the next couple of hours getting to know the young person while they are out shopping for essential items for the flat or accommodation.

The young person and his/her key worker will meet to discuss and decide on the days, times, and frequency of keyword sessions. The key worker will conduct an initial independent action assessment covering all areas of the young person’s needs as identified in the Department of Health assessment framework. This includes paying particular attention to self care and independent living skills.

The young person, in collaboration with the key worker and Social Worker, will work towards meeting the placement's planned objectives and outcomes identified in the placement plan. The young person and key worker will receive weekly and monthly progress reports from the Social Worker. The young person will have had an opportunity to read and add his/her comments to these reports before signing off on them.
